About the Role
As part of the Primary Care Networks at Stockport NHS Foundation Trust, this position plays a key role in delivering essential healthcare services across the community. Operating seven days a week, the focus is on ensuring that individuals receive the most appropriate care in a timely manner, promoting health and independence.
The District Nursing Service offers care in patients’ homes and residential facilities, addressing complex needs and supporting carers. The role requires flexibility, innovation, and a varied clinical skill set. Staff must be able to travel independently to multiple community sites to meet operational demands.
Stockport PCN is excited to provide opportunities for Band 3 Support Workers to collaborate closely with Community Nurses and Adult Social Care teams to deliver excellent service to local residents. Employees will operate under NHS terms with line management by Stockport Foundation Trust and are expected to complete the Care Certificate.
Key Responsibilities
- Work within a multidisciplinary team providing social and nursing care tailored to individual needs within patients' homes.
- Deliver delegated care following personalised care plans as directed by qualified practitioners.
- Assist patients in medication administration, including eye drops and wound care.
- Support community nursing colleagues as required.
