About the Role

The Obstetric Clinical Fellow position is based at Wexham Park Hospital, which serves a diverse and multicultural population with over 5000 deliveries annually. The consultant-led labor ward is equipped with 10 delivery rooms, a dedicated emergency obstetric theatre, plus a smaller theatre for procedures such as manual placenta removal and perineal tear repair. The unit practices a caesarean section rate of 35-38% and labor analgesia epidural rates of approximately 25-30%. Antenatal clinics for obstetric anaesthetic assessment are conducted for high-risk patients. High Dependency Unit (HDU) care is accessible either on the labor ward or nearby ICU. Additionally, a midwifery-led Birthing Centre manages low-risk pregnancies near ward 22.

Main Responsibilities

  • Administer and manage analgesia during labor, including epidurals and combined spinal-epidurals.
  • Initiate and sustain patient-controlled analgesia (PCA) using morphine (for cases like intra-uterine death) or remifentanil.
  • Provide anesthesia for emergency caesarean sections spanning categories 1 to 3, operative deliveries in theatre, and postnatal surgical procedures like manual placenta removal or perineal repair.
  • Conduct postnatal follow-ups for all patients on ward 22 who received anaesthetic interventions, documenting reviews in the EPIC electronic patient record system.

Education and Professional Requirements

  • Primary medical qualification recognized by the General Medical Council (GMC).
  • Full GMC registration with a license to practice.
  • Primary FRCA or equivalent certification.
  • At least 3 years of anaesthesia practice including on-call duties in theater, labor ward, and ICU.
  • Minimum one year of NHS anaesthetic experience at CT3/ST3 level or above.

Experience and Competencies

  • Demonstrated commitment to obstetric anaesthesia with documented logbook cases and relevant course attendance.
  • Proficient in regional anaesthesia techniques specific to obstetrics such as spinals, epidurals, and combined spinal-epidurals.
  • Experience in clinical audit, quality improvement projects, and clinical governance.
  • Strong teaching ability across specialties and grades; excellent communication and leadership skills.
  • Professionalism, self-awareness regarding clinical limits, and a willingness to seek assistance when necessary.

Desirable Qualifications

  • Awards or prizes received during training.
  • Final FRCA or equivalent credential.
  • Faculty experience on obstetric anaesthesia courses (PROMT/SimMOA).
  • Teaching qualifications.
  • Experience in developing clinical policy, research involvement including postgraduate degrees, presentations, and publications.
