Outreach Nurse - Nāhi Toro Kāinga

About the Role

Calder Health Centre, part of Auckland City Mission Te Tāpui Atawhai, is committed to providing compassionate and high-quality healthcare to all, regardless of their life circumstances. We are seeking a skilled Registered Nurse to deliver outreach services supporting individuals facing homelessness, hardship, and inequity across central Auckland. This position offers a rewarding chance to go beyond traditional clinic settings, foster trusted relationships with whānau, and enhance healthcare accessibility for people with complex health and social challenges.

Responsibilities

• Provide comprehensive nursing care in community environments, grounded in trauma-informed and culturally sensitive methods.
• Develop trusted relationships with clients and families to encourage sustained healthcare engagement.
• Collaborate closely with General Practice staff and other primary/community health professionals to enhance care access and continuity.
• Manage referrals to health, social, and community service providers, including specialists as required.
• Empower clients to gain confidence, participate actively in their health management, and make knowledgeable decisions regarding their care.
• Apply informed clinical judgment based on up-to-date nursing knowledge, evidence, and reflective practice.

Requirements

• Current New Zealand Registered Nurse Annual Practising Certificate with a minimum of 3–4 years of recent nursing experience, preferably in primary care, community health, or outreach roles.
• Demonstrated dedication to Te Tiriti o Waitangi and alignment with the Mission’s core values: Manaakitanga, Atawhai, Rangapū, Mana Tika, Mana Ōrite.
• Experience working within Māori and/or Pacific communities and supporting individuals with complex health and social needs.
• Proven ability to engage successfully with people facing mental health issues, substance use, homelessness, hardship, or significant social deprivation.
• Strong knowledge and application of trauma-informed care practices.
• Possession of a full, clean New Zealand driver’s licence.
