Job description
Role Overview
A part-time position is available within the Community Mental Health Team for Older Adults under Lancashire & South Cumbria NHS Foundation Trust, based in Barrow-in-Furness. This role is designed to support individuals with mental health issues and dementia, working alongside a multidisciplinary team in patients' homes and care homes.
Key Responsibilities
- Collaborate with senior practitioners to assess and deliver interventions aimed at individuals with complex mental health and dementia-related needs.
- Carry out therapeutic activities and interventions assigned by Community Mental Health Practitioners in community or alternative settings, focused on service users and their carers.
- Provide personalized one-to-one support to an allocated group of service users and carers to promote recovery and community living.
- Support evidence-based assessments under the supervision of Community Mental Health Practitioners to aid care planning.
- Help implement agreed care plans, monitor progress, and deliver specific interventions as directed.
- Assist in monitoring the physical health of service users.
- Maintain accurate records of patient information according to Trust policies.
Candidate Profile
- Must be committed to working with older adults with mental health needs.
- Ability to work both independently and effectively within a team environment.
- Essential frequent independent travel is required, sometimes in areas not served by public transport.
