Role overview

Berkshire Healthcare NHS Foundation Trust is inviting applications for a Community Healthcare Assistant in Windsor and Maidenhead. In this role, you will support the delivery of care to people in their own homes while working alongside experienced, supportive teams. Main purpose of the role

The Healthcare Assistant position is a key part of Community Nursing Teams. You will be expected to work to the competencies for this level, and to demonstrate the Trust’s core values through your day-to-day practice and professional behaviour.

Key duties

  • Carry out daily home visits for multiple patients.
  • Provide high-quality nursing support to patients with a broad range of health needs.
  • Deliver clinical and therapeutic care in line with Trust policies and protocols, as set out in the band 3 competency assessment tool.
  • Support holistic care planning by collecting initial information, tracking patient progress, sharing feedback and reports, and contributing to care reviews with the Community Nursing Team.
