Role Overview

As the Senior Coordinator in mental health services, you will play a central role in supporting individuals and their families to overcome the challenges of mental illness and addiction. You will lead a multi-disciplinary care team’s daily operations, ensure seamless patient transitions through the service continuum, and provide steady leadership to your colleagues.

Key Duties

• Manage care coordination, facilitating continuous, quality transitions across mental health and addiction services.
• Perform triage of referrals, assigning cases to appropriate specialists for thorough assessment.
• Identify patient care requirements, coordinate interventions, and assess outcomes autonomously, especially for complex cases.
• Lead the mental health team operationally and provide mentorship to colleagues while contributing to ongoing service development and evaluation.
• Engage in organizational initiatives, working groups, and research activities to advance service excellence.
• Promote equitable, culturally sensitive care practices across all patient interactions and team activities.
• Maintain accurate, timely documentation and ensure compliance with regulatory and professional standards.

Required Qualifications and Experience

• Current registration and valid Annual Practising Certificate as a Registered Nurse, Occupational Therapist, Social Worker, or Psychologist.
• Experience in mental health, community health, or primary care environments.
• Proven work experience with Māori and Pasifika communities.
• History of successfully coordinating care for people with complex, severe, and enduring mental health and addiction conditions.

Preferred Skills and Background

• Familiarity with both statutory and NGO mental health and addiction services.
• Experience within interdisciplinary and integrated care models.
• Background in delivering or managing care via telehealth or digital platforms.
