Programme Director for Urgent and Emergency Care Transformation

Overview

Gloucestershire Health and Care NHS Foundation Trust (GHC), as the host organization for the Gloucestershire Provider Partnership (GPP), is seeking an accomplished system leader to spearhead transformational changes in Urgent and Emergency Care across Gloucestershire.

The GPP is a collaborative alliance of Gloucestershire Health and Care NHS Foundation Trust, Gloucestershire Hospitals NHS Foundation Trust, and Gloucestershire's GP Collaborative, working together alongside the Integrated Care Board, County Council, and the Voluntary, Community and Social Enterprise sector to enhance health and care services integration, improve patient outcomes, reduce inequalities, and promote proactive community care.

Role Purpose and Responsibilities

• Take charge of implementing community-based non-elective care strategies within Gloucestershire's neighbourhoods, emphasizing a Home First philosophy and evolving intermediate care frameworks.
• Accelerate development of urgent neighbourhood services like urgent community response teams, virtual ward systems, centralized access points, and strengthen coordination with hospital emergency departments.
• Lead urgent care pathway transformations, overseeing urgent treatment centers, integrated patient flow systems, and system-wide patient transfer processes.
• Deliver strategic leadership over multiple complex programs by defining clear vision, ensuring robust governance, managing programs effectively, and achieving targeted benefits.
• Collaborate with NHS providers, primary care, local government, voluntary sectors, commissioners, and individuals with lived experience to co-create and implement sustainable care models.
• Innovate workforce and service models that enhance productivity, patient outcomes, and ensure long-term system sustainability.
• Oversee performance monitoring, quality assurance, financial tracking, and outcome reporting to provide comprehensive assurance to GPP and system boards.
• Advocate and facilitate co-production and inclusive engagement, involving patients, carers, and communities in shaping service design and delivery.
