Role overview
This opportunity is for a Psychology graduate with a solid clinical foundation who has completed both academic training and a period of supervised clinical practice. The role combines duties as an Assistant Psychologist with work as a Home Treatment Team Practitioner.
Service context
The Croydon Home Treatment Team offers urgent assessment and intensive support to people in their own homes whenever possible, as an alternative to hospital admission, or to support a quicker discharge when inpatient treatment is needed.
The post may involve working across different Trust locations depending on service needs.
Working pattern
The role is divided between two functions: 2 days per week in a Psychology Assistant capacity and 3 days per week as a Home Treatment Team Practitioner.
Home Treatment Practitioners may be rostered on either early or late shifts. The service runs from 8:00am to 10:00pm, every day of the year.
Main duties
The successful candidate will assess service users’ needs, then plan and review care in partnership with the service user, their family, and carers.
They will collaborate with the wider multidisciplinary team to support high-quality care delivery. This includes using evidence-based psychological interventions that are time-limited, carrying out risk assessment and risk management, and supporting safe discharge arrangements.
The role also includes making sure service users’ physical health needs are identified, monitored, and addressed appropriately.
Effective liaison with other professionals and services will be essential to maintain clear communication and ensure smooth transfer of care.
