Key Duties
- Provide hands-on support with activities of daily living such as transfers, bathing, grooming, toileting, and mobility while preserving client dignity.
- Keep the home environment clean and safe by changing linens, doing light laundry, and helping maintain infection control standards.
- Offer medication reminders, observe for changes in symptoms, and document vital signs or concerning observations for the supervising nurse.
- Engage clients in therapeutic activities such as reading aloud, reminiscing, range-of-motion exercises, and short walks to support overall well-being.
- Complete accurate care notes, progress updates, and concern reports in the digital charting system before the end of each shift.
Qualifications Needed
- Completion of a state-approved Home Health Aide training program and possession of an HHA certificate.
- At least 1 year of paid experience in home health or hospice care.
- A valid driver’s license and dependable transportation for client visits.
- Strong communication skills and a genuine commitment to relationship-based care.
