Role Overview

We seek a Health Coach passionate about caring for individuals with chronic health conditions like diabetes and obesity. This role demands empathy, compassion, and a dedication to catalyzing positive change both for patients and for advancing cardiometabolic health.

The Health Coach is expected to build trusting relationships through active listening and personalized support that encourages patients to believe in achievable health outcomes.

Key Responsibilities

• Deliver motivational and educational support to 9amHealth members dealing with obesity, diabetes, hypertension, high cholesterol, and weight issues via Zoom sessions.
• Employ motivational interviewing and coaching strategies to encourage sustainable behavior improvements in nutrition, sleep, physical activity, and stress management aligned with NBC-HWC standards.
• Maintain up-to-date scheduling for member appointments.
• Use member health data to tailor coaching sessions aimed at achieving individualized wellness goals.
• Accurately document all member encounters and communications in the electronic medical record system.
• Support and reinforce nutrition and lifestyle plans created by 9amHealth registered dietitians, adapting advice to member preferences and objectives.
• Identify and escalate any member issues to ensure superior care experiences.
• Provide empathetic, culturally responsive, and inclusive care respecting the diversity of all members.
• Note: Health Coaches may engage with other organizations concurrently.

Requirements

• Active NBC-HWC certification is mandatory.
• Extensive knowledge of chronic conditions such as diabetes, hypertension, dyslipidemia, and obesity.
• Minimum of five years experience in direct client care, preferably in clinical or telehealth environments specializing in chronic disease coaching.
• Comfort with digital technologies including EMRs and coaching tools.
• Strong interpersonal skills characterized by compassionate and individualized communication.
• Self-motivated, dependable, and accountable for delivering high-quality independent care.
• Experience providing virtual healthcare via chat, phone, and video platforms.
• Access to a private, quiet workspace with dependable high-speed internet capable of supporting confidential care delivery.
• Preferred qualifications include previous roles in digital health companies targeting chronic disease and multilingual capabilities to serve diverse populations.

Contractor Engagement Terms

• This position is classified as a 1099 independent contractor relationship.
• Contractors are not employees and do not receive employee benefits.
• Coaches have autonomy over when and how they deliver services, within scheduling and legal requirements.
• The engagement is non-exclusive, permitting coaches to work with multiple entities concurrently.
