Overview
The Virtual Acute Care Unit (VACU) at the Royal Berkshire Foundation Trust is a progressive team dedicated to enhancing patient care through innovative remote treatment models. This role focuses on advancing the VHS directorate by managing patients remotely, offering services between 08:00 and 20:00 every day of the week with a weekend coverage ratio of approximately 1:3.5 to 1:4.
Care Delivery Models
- Virtual Management: Utilizing remote monitoring devices, telephony, and online assessments to provide patient care from a distance.
- Face to Face: Conducting in-person clinical evaluations, administering diagnostics, imaging, invasive procedures, and intravenous therapies within the dedicated VACU clinical facilities.
- VACU@home: Providing in-home care services as part of an evolving program to extend treatment directly to patients' residences.
Daily Duties
The role involves starting the day onsite at Royal Berkshire Hospital with team handovers and operational briefings before assuming a designated patient caseload. Collaboration with a multidisciplinary team comprising consultants, physician associates, nurses, pharmacists, and clinical administrators is essential to service delivery.
Key Responsibilities
- Manage patients eligible for VACU remote care in line with service protocols.
- Work shifts across all seven days on-site at the hospital to ensure continuous care.
- Contribute to the growth and quality assurance of the VACU pathways, ensuring patient safety and effective care standards.
- Assist with patient admissions through deployment of monitoring devices and education on symptom tracking and escalation procedures.
- Use digital systems including electronic patient records and remote prescribing tools for care coordination and patient discharge back to community services.
- Engage actively in clinical governance initiatives, safety monitoring, and quality audits.
