About the Role
Based in the Alexandria, VA metro area, this full-time position offers a remote and field-based work model with a schedule from Monday to Friday plus occasional evenings and weekends. The role requires a valid driver's license with insurance and involves travel amounting to 25–50% within a 30-mile community service radius. Reporting to the Care Team Manager, the Community Health Navigator—also known internally as a Community Guide—partners with community members to foster meaningful relationships and directs them toward increased healthcare engagement, addressing social challenges impacting their wellbeing. Daily use of Reema's technology platform is integral for outreach, scheduling, and documentation. As a dynamic startup, the role adapts continually, providing opportunities for growth and innovation.
Key Responsibilities
- Perform both phone and in-person outreach to enroll individuals in the care program.
- Manage a caseload ranging from 80 to 120 members, providing backup support for colleagues when needed.
- Evaluate members' needs, including social determinants of health, to create personalized care plans.
- Build trustful relationships and follow up promptly and appropriately.
- Support members in understanding the importance of health activities, navigate hesitations empathetically, and facilitate completion.
- Help members progressively overcome barriers to healthcare achieving measurable progress monthly.
- Coordinate referrals and assist with scheduling and attending appointments.
- Plan and prioritize tasks independently to meet monthly goals and daily performance measures.
- Engage members via multiple outreach modes such as phone, text, email, and in-person visits.
- Connect members to community and health-related resources, including transport or accompaniment if necessary.
- Maintain confidentiality and professional boundaries; collaborate with multidisciplinary teams on complex cases.
- Ensure timely and thorough documentation within the Reema application, keeping care plans updated.
- Adhere strictly to HIPAA and patient confidentiality regulations.
- Operate flexibly and proactively in a fast-evolving startup environment, appreciating change as an opportunity.
- Represent the organization by providing respectful service and fostering healthy relationships in the community.
Required Qualifications and Attributes
- Strong orientation toward solution-finding to overcome obstacles without delay.
- Effective verbal and written communication skills for clear problem explanation and requests.
- Proficiency and adaptability with both mobile and desktop technology platforms.
- Self-motivated with the ability to independently manage a remote caseload.
- Committed to teamwork and maintaining connections in a remote setting, including attendance at occasional in-person meetings.
- At least 18 years of age and available during core weekday business hours.
- Valid driver’s license, insurance, and dependable transportation if field-based.
- Passing pre-employment drug screening and background check is mandatory.
- Capability to lift and carry up to 25 pounds occasionally.
Preferred Experience
- One or more years working directly with the served community or equivalent experience.
- Bachelor's degree or equivalent experience in Social Work, Human Services, Psychology, Health Sciences, or related disciplines.
- Familiarity with crisis management, case coordination, and maintaining professional boundaries in community environments.
- Skilled in relationship-building and knowledgeable about community resources and healthcare navigation.
- Experience collaborating within multidisciplinary teams.
