Job description
Position Summary and Location
This full-time Community Guide role, based in the Charleston, SC area, operates on a hybrid work model and reports to the Senior Care Team Manager. The position requires a commitment to improving health and wellness for individuals facing complex social and health challenges by working closely with community members and resources.
Primary Responsibilities
- Engage and enroll members from diverse ethnic backgrounds, focusing on those with complex health and social needs, through outreach and ongoing support.
- Develop comprehensive, member-centered care plans by assessing individual needs, including social determinants of health.
- Build trusted relationships with members, provide meaningful and consistent support, and assist them in achieving their health and wellness goals.
- Coordinate referrals and facilitate appointment scheduling and attendance across behavioral health, primary care, and other services.
- Manage a caseload of 60 to 100 members, including coverage during the absence of other Community Guides.
- Prioritize workload effectively to meet monthly targets and utilize various communication methods (in-person visits, phone, text, email) aligned with member preferences.
- Provide in-person visits as needed, including home, community, and treatment settings; transport and accompany members to services when necessary.
- Ensure thorough and timely documentation of all member interactions and care plans using Reema's proprietary app, adhering to HIPAA and confidentiality regulations.
- Represent Reema Health positively, working collaboratively within a fast-paced startup environment and engaging with community resources.
Qualifications and Experience
- Bachelor’s degree in relevant fields such as Social Work, Human Services, Psychology, or Health Sciences, or equivalent related experience.
- At least three years of direct experience working with the target population and community services.
- Strong commitment to working with individuals needing complex health support, with experience in crisis planning, case management, and safety protocols.
- Highly developed skills in critical thinking, time management, multitasking, and strategic planning.
- Excellent relationship-building skills to foster meaningful connections and initiate engagement.
- Technologically proficient with Google Workspace and capable of quickly adapting to new digital tools and troubleshooting technical issues independently.
- Valid Driver’s License, insurance, and reliable transportation required, with the ability to travel 25-50% within a 25-mile radius.
- Must be 18 years or older; contingent on passing background check and pre-employment drug screening.
Work Schedule and Physical Requirements
- Workweek is Monday through Friday with occasional evening or weekend hours as needed.
- Ability to lift and carry up to 25 pounds occasionally.
