Community Guide - Community Health

About the Role

Reema Health is seeking a dedicated Community Guide to join their mission-driven team aiming to transform healthcare and social care integration. Based in Columbia, SC, this full-time position blends in-person fieldwork and remote engagement. The Community Guide builds meaningful relationships with members facing complex health and social challenges, supporting them through personalized outreach, care coordination, and empowerment strategies.

Key Responsibilities

• Engage and enroll members from diverse backgrounds with complex social and chronic health issues.
• Develop member-specific care plans considering social determinants of health and facilitate referrals to behavioral health, primary care, and community resources.
• Manage an active caseload of 60-100 members through phone, text, email, and face-to-face visits including at homes, community sites, and hospitals.
• Provide empathetic support while maintaining confidentiality and professional boundaries, consulting multidisciplinary teams as needed.
• Use the Reema app to document interactions and update care plans promptly and accurately, complying with HIPAA regulations.
• Adapt and prioritize workload independently to meet monthly objectives and succeed in a dynamic startup environment.
• Build and maintain positive relationships with community resources to enhance member support, occasionally assisting with transportation.
• Prepare members for service graduation by encouraging independence and self-sufficiency.
• Represent Reema Health professionally and collaboratively engage with internal teams and community partners.

Required Qualifications and Skills

• Bachelor’s degree in Social Work, Human Services, Psychology, Health Sciences, or related field; or equivalent experience.
• At least three years of direct experience working with populations facing complex health and social challenges, with local community knowledge preferred.
• Strong skills in crisis planning, case management, boundaries, personal safety, and problem-solving.
• Excellent verbal and written communication skills and ability to cultivate meaningful relationships.
• Proficient with Google Workspace and comfortable navigating web-based platforms, mobile and desktop applications.
• Ability to manage multiple tasks and independently troubleshoot technical and operational issues.
• Valid driver’s license, reliable transportation, vehicle insurance, and ability to travel 25-50% within a 25-mile radius.
• Capable of lifting and carrying up to 25 pounds occasionally.
• Must pass background check and drug screening; comply with HIPAA and federal/state regulations.

Working Conditions and Schedule

This exempt position offers a hybrid work model with primary activities occurring onsite in Columbia, SC, and includes Monday to Friday schedules with occasional evenings and weekends as needed. The role requires substantial field visits and community interaction.

Compensation and Benefits

• Annual salary range: $50,000 to $54,000
