About Reema Health and Role Overview

Located in Greenville, SC, Reema Health offers a community-centric, personalized care approach aimed at simplifying access to essential services for individuals facing health and social challenges. As a start-up, the company focuses on integrating health and social care to improve member outcomes through relationship-building and resource navigation.

The Community Guide role is a full-time position reporting to the Senior Care Team Manager, requiring a hybrid work model with a schedule mainly from Monday to Friday, occasionally involving evenings or weekends, and includes 25-50% travel within a 25-mile community service radius. A vehicle with proper licensing and insurance is mandatory.

Key Responsibilities

  • Engage individuals from diverse ethnic backgrounds dealing with complex social and chronic health conditions, enrolling them into Reema's care programs.
  • Conduct initial outreach, build trusting relationships, and develop member-centered care plans that address social determinants of health.
  • Coordinate and facilitate access to healthcare and behavioral services, including scheduling appointments and follow-ups.
  • Manage an active caseload of 60-100 members, providing support and coverage for colleagues when necessary.
  • Effectively prioritize and organize workload to meet monthly organizational goals.
  • Communicate empathetically, maintain professional boundaries, and seek collaboration for complex situations.
  • Provide in-person visits in members' homes, community settings, hospitals, or treatment centers as needed.
  • Utilize various communication methods—text, phone, email, or face-to-face—matching members’ preferences.
  • Identify and connect members with appropriate community and health resources, including transportation assistance when necessary.
  • Prepare members for the conclusion of services, promoting self-sufficiency and independence.
  • Maintain precise documentation and update care plans promptly using Reema’s technology platforms, ensuring compliance with HIPAA and confidentiality standards.
  • Represent Reema Health positively by adapting to a dynamic startup environment, working both independently and collaboratively.

Candidate Requirements and Qualifications

  • Bachelor’s degree in Social Work, Human Services, Psychology, Health Sciences, or related fields, with relevant experience substitutable for education.
  • Minimum of three years’ direct experience working with communities facing complex health and social challenges, possessing local community knowledge.
  • Passion for assisting individuals with complex health needs who embrace recovery.
  • Experience in crisis planning, case management, and ensuring safety and professional boundaries during community interactions.
  • Strong critical thinking, multitasking, time management, and relationship-building skills.
  • Ability to develop, adapt, and execute outreach and care plans effectively.
  • Proficiency in Google Workspace tools and comfort using various web-based applications and mobile platforms.
  • Demonstrated independence in troubleshooting technical issues related to connectivity, apps, and devices.
  • Must be at least 18 years old, have reliable transportation with a valid driver’s license, insurance, and a clean driving record.
  • Successful completion of pre-employment drug screening and background checks required.
  • Capability to lift and carry up to 25 pounds occasionally.
