Job Overview

This role involves providing personalized healthcare and maintenance assistance to clients in their residences, focusing on various support activities such as transferring, feeding, mobility assistance, and prescribed exercises.

Client Interaction

Key Responsibilities

• Deliver care tasks aligned with the Home Health Aide Plan of Care (POC).
• Maintain open and effective communication with administrative personnel concerning client needs and changes in condition.
• Accurately record all care provided according to the designated care plans.
• Participate actively in team meetings to support care continuity and collaboration among care team members.
• Implement infection control practices and universal precautions to safeguard clients and staff.
• Commit to professional development by engaging in assigned in-services and training promptly.
• Fulfill all mandatory agency and licensing board continuing education requirements.

Experience and Education Requirements

• At least one year of experience as a home health aide or certified nursing assistant within a home health setting; formal nursing arts training may serve as equivalent experience as permitted by state regulations.
• High school diploma or equivalent qualification.

Certifications and Licenses

• Valid Home Health Aide certificate or similar certification recognized by the state.
• Completion certificate from an HHA/CNA training program that meets Health and Human Services standards.
• Current CPR certification.
• Possession of a current driver's license and reliable transportation with appropriate auto liability insurance, ensuring the ability to travel locally and work flexible hours.

Physical and Environmental Conditions

• Capable of prolonged standing and walking, with ability to lift or transfer up to 50 pounds.
• Work may involve stressful scenarios requiring quick decisions and resourcefulness.
• Exposure to various home environments, including potential contact with blood-borne pathogens and infectious materials.
• Possible need to respond to emergency calls and occasional adverse weather conditions.
