Role Overview
Reema Health is looking for a self-driven, experienced Community Guide to support its community-based care model in New York City. The role is part of a mission-led startup focused on improving access to health and social care by creating real human connection through technology and outreach.
This position supports members through short-term, high-volume engagement. The Community Guide builds authentic relationships, helps members access resources, and uses a digital platform to connect by phone, text, and scheduled community visits while documenting all interactions accurately.
Work Location and Schedule
Location: Virtual, with a requirement to be local to NYC.
Work model: Hybrid.
Job status: Full time.
FLSA status: Exempt.
Schedule: Monday through Friday, with occasional evenings and/or weekends.
Vehicle required: No.
Travel required: 5%.
Reporting line: Care Team Manager.
Key Responsibilities
The Community Guide is responsible for outreach, member engagement, resource connection, documentation, and cross-functional collaboration. The role requires comfort with ambiguity, strong communication, and the ability to manage a sizable caseload while meeting performance goals.
Requirements and Qualifications
Candidates should have a bachelor’s degree in Social Work, Human Services, Psychology, Health Sciences, or a related discipline, though equivalent related experience may be considered in place of the degree requirement. A minimum of two years of direct experience with the served population or community is required, along with familiarity with the community itself.
Strong working knowledge of crisis planning, case management, professional boundaries, and personal safety in community-based work is important. The role also calls for excellent critical thinking, time management, prioritization, multitasking, relationship-building, and outreach execution skills. Comfort with Google Workspace, web-based tools, and quick troubleshooting of basic technical issues is expected.
Applicants must be 18 years of age or older and able to comply with all applicable confidentiality and HIPAA requirements. The position involves daily use of technology-based tools and may require lifting and carrying up to 25 pounds occasionally.
Responsibilities in Detail
- Reach out to community members, engage them in the program, and support enrollment.
- Deliver meaningful, high-quality assistance that helps members improve their health and overall wellbeing.
- Build trusted relationships and follow through consistently to help members progress toward their goals.
- Prospect through cold outreach and manage a caseload of about 100 to 125 members, while also stepping in to support other Community Guides when coverage is needed.
- Track monthly goals and metrics, then plan and prioritize daily work independently to meet targets.
- Listen with empathy, respond with professionalism, protect boundaries, and involve support resources when situations become complex.
- Communicate through phone, text, and email in the way that best matches each member’s preference.
- Connect members with appropriate community and health resources and build working relationships with those resources as needed.
- Help members move toward independence and prepare them for graduation from the program.
- Record all member interactions and work activity thoroughly and promptly in the Reema App.
- Maintain strict confidentiality and follow HIPAA standards for protected health information.
- Work both independently and with others in a fast-paced startup setting where change is expected.
- Treat diverse members with dignity and respect while building constructive relationships with community partners and customers.
- Collaborate with teammates across the organization and contribute perspective while learning from others.
- Accept that responsibilities may expand or change as business needs evolve.
Education, Experience, and Technical Skills
- Bachelor’s degree in a related field such as Social Work, Human Services, Psychology, or Health Sciences is preferred, with relevant experience allowed to substitute for some education.
- At least 2 years of direct experience working with the relevant community or population.
- Knowledge of crisis planning, case management, boundaries, and safety practices in field/community work.
- Ability to think critically, manage time well, plan outreach strategically, and juggle multiple tasks.
- Strong drive to meet goals, metrics, and targets.
- Ability to build and sustain relationships quickly and effectively.
- Comfort starting conversations and establishing rapport in a short time.
- Ability to design, adjust, and carry out outreach plans.
- Fluency with Google Workspace tools including Gmail, Docs, Sheets, Slides, Drive, and Meet.
- Ease working with web-based systems across mobile and desktop browsers.
- Ability to troubleshoot basic technical issues, such as connectivity, app behavior, and device performance, and escalate when necessary.
