Role Overview

As a licensed Clinical Pharmacist on Arine's Clinical Care team, you will use the company’s software platform to enhance patient outcomes through medication therapy management and clinical services. The role involves significant patient interaction via phone and requires expert medication consultations, adherence counseling, and collaboration with providers and pharmacies. Under a Pharmacist Lead’s supervision, you will also contribute to platform improvements and support clinical team members.

Key Responsibilities

  • Conduct phone-based Medication Therapy Management including comprehensive and targeted medication reviews, adherence counseling, medication reconciliation, and education.
  • Review medication reviews and counseling completed by other pharmacists.
  • Analyze and document patient data using Arine’s software and related tools, preparing detailed verbal and written reports on patient care and progress.
  • Coordinate with healthcare providers, pharmacies, and patients for timely resolution of medication issues, especially during care transitions.
  • Develop interventions to optimize medication use, enhance quality measures, and meet defined goals.
  • Maintain productivity and quality standards in managing patient, provider, and pharmacy communications.
  • Adapt to varied clinical projects and priorities as business needs evolve.
  • Identify workflow and technology improvements by cross-functional collaboration and driving automation.
  • Ensure effective daily communication and patient care continuity through clear handoffs.
  • Mentor clinical team members and train new users on workflows and platform usage.

Qualifications and Experience

  • Doctor of Pharmacy (PharmD) from an accredited institution.
  • Active, good-standing pharmacist license (multi-state licensure advantageous).
  • Familiarity with CMS quality measures, Star Ratings, HEDIS, Medicare Part C and D medication processes.
  • Extensive knowledge of chronic diseases (cardiology, pulmonology, endocrinology, behavioral health) and polypharmacy management.
  • Deep understanding of prescription and non-prescription drugs, their effects, contraindications, and dosage considerations.
  • Ability to evaluate prescriptions for appropriateness considering patient-specific factors.
  • Proficient in motivational interviewing and patient adherence strategies.
  • Strong communication skills to engage diverse patient populations compassionately and effectively.
  • Proven reliability, accountability, and high attention to detail in documentation.
  • Comfort with technology, including Google Workspace, Mac OS, collaboration tools (Slack, Five9), and multitasking across browser-based systems.
  • Willingness to obtain additional state licenses as needed.
  • Ability to work 40 hours per week within Monday–Friday 6AM–6PM Pacific Time schedule, including up to two rotating Saturdays monthly.

Preferred Additional Experience

  • Ambulatory care, especially chronic disease management.
  • Inpatient, hospital, or emergency pharmacy with direct clinical interactions.
  • Geriatric or long-term care pharmacy experience.
  • Prior work with MTM platforms and health plan or managed care pharmacy settings.
  • High-volume healthcare or pharmacy environments such as retail or call centers.
  • Board certifications, residency training, or virtual healthcare experience.
  • Familiarity with various electronic medical records systems.

Work Environment and Requirements

  • Private home workspace ensuring information security and patient privacy.
  • Reliable internet with low latency capable of handling video and voice calls.
  • Attendance at occasional on-site meetings for onboarding and team collaboration.
  • Ability to pass a background check and work legally in the United States.

Compensation and Benefits

This fully remote, part-time temp-to-hire role offers an hourly wage between $57 and $65, dependent on experience and other factors. The expected contract duration is six months with potential extension or conversion to permanent employment. Full employee benefits begin immediately upon hiring. Seasonal Work Schedule

The Clinical Care team follows a seasonal workload, peaking from May to early December with increased demands and limited time-off. Optional overtime may be available during this period. An annual department shutdown occurs for 2–3 weeks during late December to early January, followed by a modified schedule through February. Candidates should be comfortable working in this dynamic environment.
