Job Overview

This part-time remote role invites a Licensed Registered Nurse to support patients in managing medications, particularly for heart disease and diabetes. The position involves patient outreach, medication reconciliation, education, and collaboration with pharmacists and healthcare providers to enhance treatment adherence and reduce hospital readmissions. The nurse will also support compliance with quality measures such as HEDIS and CMS documentation while contributing to workflow improvements and patient care coordination.

Responsibilities

• Conduct post-discharge follow-up calls to reconcile medications and clarify regimens
• Educate patients to promote medication adherence and manage conditions like heart disease and diabetes
• Collect blood pressure readings and assist in meeting quality goals
• Partner with pharmacists and care providers to resolve medication-related concerns affecting patient outcomes
• Ensure continuity of care across healthcare teams and maintain detailed documentation in proprietary software
• Coordinate with hospitals, clinics, and pharmacies for prompt care issue resolution
• Triaging high-risk patients for medication review and side effect management
• Engage in process improvements aimed at enhancing care coordination and outcomes
• Help obtain necessary referrals and authorizations within care management protocols
• Manage inbound and outbound communications while achieving performance targets
• Set up telephonic or virtual appointments with clinical pharmacists as needed
• Follow prescribed workflows, adapt to evolving processes, and take ownership of assigned tasks

Requirements

• Bachelor's Degree in Nursing (BSN) and an active Registered Nurse license in good standing (multi-state licenses advantageous)
• Previous case management experience and comprehensive knowledge of care coordination
• Strong expertise in medication management, reconciliation, and terminology
• Excellent communication skills for patient education and support
• Proficient computer skills, including Google Workspace, Mac OS, and contact center technologies (Slack, Five9)
• Ability to multitask, prioritize workload, and work efficiently in fast-paced environments
• Dependability, accountability, and attention to detail with accurate documentation practices
• Experience working in metrics-driven settings focused on outcomes and quality
• A team-oriented approach balanced with independent initiative
• Willingness to accept feedback and engage in continuous learning
• Availability to work up to 40 hours per week, Monday through Saturday until 7 PM Pacific Time

Preferred Qualifications

• Familiarity with electronic medical records (EMR) systems
• Experience in ambulatory care settings, medication histories, and transitions of care
• Knowledge of pharmacy operations and medication therapies
• Certified Diabetes Educator (CDE) certification

Compensation and Schedule

This temp-to-hire position typically lasts six months with potential extension or permanent placement based on performance and organizational needs. Full benefits commence immediately from the first day. The anticipated pay rate ranges between $39 and $60 per hour, depending on experience, role level, and location.

The Clinical Care team operates seasonally with increased workload from May to early December, including limited time-off and optional overtime. There is a 2-3 week department shutdown between late December and early January, after which operations resume on modified schedules until February.
