The Behavioral Health Care Team Manager plays a critical leadership role in this mission, managing a hybrid care team that partners closely with clinical staff to deliver holistic non-clinical support. This role demands adaptability, independent problem-solving, and effective collaboration to foster team engagement and meet Reema’s care objectives.
Primary Responsibilities
- Lead and manage behavioral health Community Guides, ensuring clear communication of program goals, team metrics, and Reema values.
- Drive change management initiatives and guide teams through innovation and organizational shifts within a fast-growing startup environment.
- Develop operational structures to promote communication, accountability, and professional development such as regular one-on-one meetings, goal setting, team gatherings, case reviews, and community events.
- Monitor Guide performance actively and provide consistent, constructive feedback and recognition.
- Collaborate with clinical psychotherapists and prescribers to align services and workflows, ensuring integrated, high-quality member care.
- Conduct weekly staffing, metric updates, and goal-setting sessions with the Director of Clinical Care Team Operations.
- Provide direct supervision and support to Guides including resource research and outreach partnership development.
- Take part in recruitment processes and facilitate onboarding plus ongoing training for new and existing staff.
- Ensure strict adherence to confidentiality, HIPAA standards, and data security protocols across the team.
- Establish and nurture relationships with local community partners in mental health care continuums.
Direct Member Engagement
- Engage members through empathetic, trauma-informed outreach via phone, text, home visits, and face-to-face contact to build trust and deliver support.
- Maintain thorough knowledge of local community resources, coordinate referrals, follow-ups, and accompany members to appointments when needed.
- Offer comprehensive, ongoing support through crisis situations and everyday challenges, encouraging member independence and autonomy.
Qualifications and Experience
- Bachelor’s degree in Social Work, Human Services, Psychology, Health Sciences, or related field (or equivalent relevant experience).
- At least three years of supervisory experience in healthcare, mental health, social work, or community-based settings.
- Minimum five years working directly with the local community affected by serious mental illness, chemical health issues, or similar challenges.
- Proficient critical and strategic thinking, with strong time management and prioritization skills.
- Familiarity with crisis planning, case management, behavioral health, and multidisciplinary teamwork.
- Experience supporting diverse populations, including adolescents, homeless individuals, and those difficult to engage.
- Excellent relationship building, communication skills, and ability to quickly establish connections and outreach plans.
- Comfortable working with technology, particularly Google Workspace, web-based platforms, and troubleshooting digital tools.
- Ability to navigate ambiguity and innovate in fast-paced startup environments.
Additional Requirements
- Must be 18 years or older.
- Reliable transportation with valid driver's license, insurance, and clean driving record; role requires occasional travel within Texas and a personal vehicle.
- Must successfully pass drug screening and background checks.
- Ability to lift up to 25 pounds occasionally.
- Commitment to maintaining patient confidentiality per HIPAA regulations.
Work Schedule and Conditions
This full-time role operates on a hybrid work model with a Monday to Friday schedule, occasionally requiring evening or weekend availability. The position includes domestic travel within Texas.
