About the Role
The Social Worker at the Family Service Centre (FSC) in Admiralty is responsible for providing comprehensive casework, counselling, and community assistance to vulnerable individuals and families. This position entails conducting thorough assessments, designing intervention plans, managing active cases, and collaborating with various community partners to ensure clients receive holistic support, all aligned with social work principles and ethics.
Key Responsibilities
- Handle case referrals efficiently.
- Manage approximately 30 active counselling and casework cases each quarter, mainly Group 2 cases.
- Perform assessments, plan interventions, and implement suitable strategies for client cases.
- Organize and participate in case conferences with partner agencies.
- Attend networking events and case management meetings to coordinate care.
- Maintain thorough documentation and assess case outcomes.
- Develop and periodically review intervention approaches.
- Submit timely reports related to case progress.
- Refer clients to relevant agencies for additional support and services.
- Conduct intake assessments for new clients as part of the Intake & Triage Team adhering to established social work practice standards.
- Engage with community organizations and stakeholders to build networks and assess community needs.
- Contribute to planning and delivering one of the Group Work programs conducted at the FSC.
Qualifications and Experience
- Possession of a Degree or Graduate Diploma in Social Work.
- Fresh graduates in Social Work are encouraged to apply; prior internship or placement experience in the social work field will be considered an advantage.
- Strong foundation in social work values, ethics, and practices.
- Familiarity with working alongside vulnerable groups, particularly low-income families.
