About the Role
H2 Homecare is seeking committed Registered Nurses (RN) or Licensed Practical Nurses (LPN) passionate about providing pediatric care in patients' homes throughout Georgia. This nurse-owned healthcare organization prioritizes client wellness and education, supporting families medically, physically, and spiritually. Nurses at H2 Homecare are essential to delivering quality, safe care with collaboration and compassion.
Key Responsibilities
- Recognize and document changes in patient condition using appropriate clinical forms with timely resolution.
- Notify physicians to secure necessary orders for interventions in accordance with state guidelines.
- Administer treatments and medications strictly following physician directives.
- Maintain documentation that reflects patient and family involvement in care planning.
- Complete accurate time logging and documentation via the Alayacare electronic medical record system.
- Communicate significant patient status changes to the Director of Clinical Services or Clinical Supervisor/Case Manager.
- Recommend order modifications based on clinical assessment and nursing judgment.
- Document and report all physician interactions and care coordination within medical records as needed.
- Abide by H2 Homecare’s policies and procedures.
Qualifications and Requirements
- Active license as a Licensed Practical Nurse or Licensed Vocational Nurse in Georgia.
- Graduated from an accredited practical or vocational nursing program.
- At least one year of professional nursing experience; homecare experience is advantageous.
- Certification in CPR currently valid.
- Clean background check is mandatory.
