Open Nurse (LPN) - Pediatric Homecare

About the Role

H2 Homecare is seeking committed Registered Nurses (RN) or Licensed Practical Nurses (LPN) passionate about providing pediatric care in patients' homes throughout Georgia. Key Responsibilities

• Recognize and document changes in patient condition using appropriate clinical forms with timely resolution.
• Notify physicians to secure necessary orders for interventions in accordance with state guidelines.
• Administer treatments and medications strictly following physician directives.
• Maintain documentation that reflects patient and family involvement in care planning.
• Complete accurate time logging and documentation via the Alayacare electronic medical record system.
• Communicate significant patient status changes to the Director of Clinical Services or Clinical Supervisor/Case Manager.
• Recommend order modifications based on clinical assessment and nursing judgment.
• Document and report all physician interactions and care coordination within medical records as needed.
• Abide by H2 Homecare’s policies and procedures.

Qualifications and Requirements

• Active license as a Licensed Practical Nurse or Licensed Vocational Nurse in Georgia.
• Graduated from an accredited practical or vocational nursing program.
• At least one year of professional nursing experience; homecare experience is advantageous.
• Certification in CPR currently valid.
• Clean background check is mandatory.
