Medical Social Worker (Homecare)

Job Overview

We are seeking a compassionate Medical Social Worker to join our homecare team, providing psychosocial and emotional support to patients and their caregivers. The successful candidate will take a lead role in advance care planning discussions and support patients through various stages of care.

Key Responsibilities

• Deliver psychosocial and emotional care to patients and their families or caregivers.
• Lead advance care planning (ACP) discussions and complete Preferred Care Plans.
• Offer bereavement support to families affected by patient loss.
• Coordinate financial assistance programs for patients in need within the organization.
• Arrange medical escort and transportation services for patients.
• Refer patients appropriately to respite care, nursing homes, and other required services.
• Provide counseling and mediation services to patients and families.
• Perform crisis intervention when necessary.
• Engage in educational activities, training, and professional networking.

Job Requirements

• Possess a degree or postgraduate diploma in Social Work.
• Preferably have a minimum of two years relevant work experience.
• Demonstrate a strong interest in working with elderly individuals.
• Exhibit compassion and maintain a positive, caring demeanor towards patients and internal and external clients.
• Have excellent communication, interpersonal, and problem-solving abilities.
• Be detail-oriented, diligent, and effective as a team player.
• Show adaptability and comfort with changes in work environment or processes.
