Position Summary
Sequoia Transitional Care is looking for a highly organized and knowledgeable Medical Records Director responsible for overseeing the accuracy, security, and regulatory compliance of resident health records at its skilled nursing facility in Sequoia Park, WA. This role collaborates closely with clinical and administrative teams to ensure health documentation is accurate, timely, confidential, and ready for audits and inspections.
Key Responsibilities
- Administer and oversee the creation, maintenance, organization, and proper retention of resident medical records.
- Ensure all records adhere to HIPAA, federal and state mandates, and internal facility policies.
- Maintain precise documentation concerning resident admissions, discharges, transfers, and clinical updates.
- Regularly audit medical records for completeness, accuracy, appropriate authorizations, and timeliness, following up as necessary with relevant departments.
- Collaborate with nursing, therapy, social services, business office, and administrative teams to support their documentation needs.
- Manage medical record requests from authorized individuals including residents, family members, legal representatives, outside healthcare providers, and governmental agencies, ensuring proper authorization is obtained before disclosure.
- Supervise electronic health record (EHR) systems, assisting personnel with documentation or system access issues.
- Generate reports, conduct audits, and prepare documentation for internal reviews, external surveys, and compliance inspections.
- Safeguard the confidentiality and integrity of sensitive resident and facility health information.
- Maintain preparedness for regulatory surveys and respond to inquiries about documentation.
- Organize archived records in line with required retention schedules.
- Provide management, training, scheduling, and performance oversight to medical records staff, ensuring compliance with applicable regulations and policies.
Qualifications and Experience
- Associate's or Bachelor's degree in Health Information Management or a related discipline preferred.
- Registered Health Information Technician (RHIT) certification or equivalent credential is preferred.
- A minimum of two years’ experience in medical records or health information management is preferred.
- Prior work experience in skilled nursing, post-acute, or long-term care settings is strongly favored.
- Deep understanding of HIPAA, Medicare and Medicaid documentation standards, and regulations governing skilled nursing facilities.
- Proficiency in electronic health record systems and Microsoft Office applications.
- Exceptional organizational, communication, analytic, and problem-solving capabilities.
- High attention to detail enabling precise and complete record-keeping.
- Skill in managing multiple tasks effectively within deadlines.
- Demonstrated professionalism and discretion in handling confidential health information.
Physical and Work Environment
- Tasks primarily involve extended sitting, typing, filing, and reviewing both electronic and paper records.
- Occasional periods of walking, standing, bending, reaching, with the ability to lift items up to 25 pounds.
- Effective functioning in a busy healthcare setting is required, with accommodations available for qualified individuals with disabilities.
