Job description

Role overview

This position is responsible for managing the release of patient health information from medical records in line with hospital procedures, applicable regulations, and federal and state laws. The role also supports the accurate and timely completion of electronic death certificates through the Electronic Death Registration System, following guidance from the New York City Department of Health and Mental Hygiene and Health Information Management.

Key duties

  • Apply hospital policies on disclosure of health information and ensure requests are handled correctly.
  • Review incoming correspondence and respond in accordance with hospital, governmental, and regulatory requirements.
  • Receive mail, date-stamp it, and route it for processing.
  • Enter required details into the computerized system according to hospital procedures.
  • Coordinate with physicians and other departments when additional information is needed.
  • Prepare records for release by copying the needed sections or producing electronic copies.
  • Assess and collect applicable fees where required.
  • Scan authorization forms and maintain related records.
  • Examine subpoenaed records, screen them appropriately, and prepare paper or electronic copies.
  • Assist walk-in patients or visitors requesting their medical records.
  • Retrieve and duplicate records requested by the Business Office.
  • Create registration profiles for physicians who complete the electronic death registration process.
  • Support physicians in completing the fields required by the Department of Health.
  • Track the status of death certificates, generate reports, and follow up to ensure timely electronic submission.
  • Monitor the completion of autopsy consent forms and report on status.
  • Coordinate with the hospital morgue when a family agrees to an autopsy.
  • Liaise with funeral homes and provide updates during the death certificate completion process.
  • Carry out other related assignments as directed by management.

Qualifications

A high school diploma or GED is required. An associate degree in Health Information Technology or another related health care field is preferred, and may be accepted in place of the stated experience requirement. Candidates must have knowledge of medical terminology, basic anatomy and physiology, and medico-legal as well as regulatory requirements for records management. Three years of medical records experience in a health care setting is required, with prior use of an electronic medical record preferred. RHIT or RHIA certification is preferred.

This role is part of the SEIU 1199-MSH collective bargaining unit. The position is based at Mount Sinai Hospital in New York, NY and is scheduled Monday through Friday from 9:00 AM to 5:00 PM. Compensation

The hourly pay range for this position is $27.8789 to $29.9076. Actual pay may vary depending on experience, education, and operational need. Bonus pay, differentials, incentives, and benefits are not included in the stated range.
