Role overview

This remote opportunity is for a Medical Assistant Patient Care Coordinator based in the United States and is being handled by a partner employer. The role focuses on steady, patient-centered coordination for people living with chronic conditions. You will act as a primary contact for a sizable patient panel, helping ensure regular follow-up, care-plan adherence, and timely support. The work blends clinical understanding, coaching, communication, and organization to help close care gaps and improve outcomes. You will work closely with providers, clinical teams, and caregivers, while maintaining accurate documentation and operating in a HIPAA-compliant environment.

What you will do

  • Oversee a patient panel of about 400 to 450 individuals and complete monthly outreach to support continuity of care and care-plan follow-through.
  • Make 50 to 60 outgoing patient calls each day, offering health coaching and individualized support.
  • Partner with providers, clinical staff, and caregivers to recognize medical and social needs and help arrange the right interventions.
  • Contribute to care programs including Chronic Care Management, Behavioral Health Integration, and Remote Physiological Monitoring.
  • Represent the patient’s voice by listening carefully, resolving concerns, and keeping communication flowing across the care team.
  • Record patient interactions promptly and accurately in the designated care coordination systems.
  • Follow HIPAA rules and internal quality expectations in all communications and recordkeeping.

Requirements

  • Hold a nationally recognized Medical Assistant credential such as CMA, RMA, CCMA, NCMA, CMAC, or an equivalent MA-C, or be a Certified Pharmacy Technician (CPhT).
  • Bring at least 1 year of clinical experience in a healthcare setting such as primary care, a hospital, or a specialty practice.
  • Have hands-on experience with EMR systems and strong general computer skills.
  • Be able to move efficiently between multiple healthcare and communication tools in a fast-paced setting.
  • Communicate clearly, build trust with patients, educate them effectively, and collaborate well with care teams.
  • Stay organized, pay close attention to detail, and manage high patient volumes and outreach goals comfortably.
  • Be authorized to work in the United States and maintain a private, HIPAA-compliant home workspace with dependable internet access.
  • Preferred experience includes care coordination, chronic care management, RPM programs, or behavioral health integration; bilingual English/Spanish ability is an added advantage.
