Role overview
NHS Ayrshire & Arran is hiring an experienced registered nurse to step into a Nurse Team Lead role within the Home First service. This is a permanent Band 7 position working 36 hours per week (1 WTE) across multiple localities.
Home First is built around a joined-up, community-first model of care. It works closely with acute and community teams to help people return home or remain in a homely setting wherever possible. The service focuses on prevention, early anticipation of need, and helping people manage their own health and wellbeing with support.
The team operates in partnership with Hospital at Home, District Nursing, Community Allied Health Professionals, Adult Social Work, and Adult Social Care to provide a coordinated community response across the Borders.
What the role involves
The successful candidate will lead and support Home First Nurse Coordinators and Health Care Support Workers in delivering short-term reablement and rehabilitation aimed at restoring function, independence, and reducing the likelihood of long-term care needs.
You will carry overall responsibility for designing, putting into practice, and reviewing reablement and nursing care programmes for people receiving care in their own homes.
This post requires a strong, confident decision-maker who can work independently and provide direct line management to both registered and unregistered staff.

The role supports service delivery across Berwickshire, Central, Cheviot, Teviot and Tweeddale, so a full UK driving licence is required to travel across the wider service area as needed.
Working patterns include weekdays and weekends on a rota basis to meet operational requirements.
