Fundamentals of Care Improvement and Practice Facilitator

Role Overview

This position offers the chance to collaborate closely with the Fundamentals of Care Lead Nurse/AHP/Midwife in advancing the Trust's Fundamentals of Care Strategy and launching the Fundamentals of Care Academy. The role focuses on providing practical, visible support throughout clinical, community, and outpatient services to promote safe, high-quality, compassionate, and patient-focused care. Key activities include supporting quality improvement efforts, delivering education and coaching, conducting audits, ensuring assurance, and leveraging data to promote lasting improvements in care quality and patient outcomes.

Key Responsibilities

The post holder will assist the Fundamentals of Care Lead Nurse/AHP/Midwife, alongside the Quality and Safety Lead and Corporate Director of Nursing, to embed the Fundamentals of Care Strategy and Academy across the Trust. This includes offering hands-on practice support, education, coaching, and facilitating quality improvement initiatives to help clinical teams consistently deliver effective and compassionate care. Establishing and maintaining strong working relationships to support high-quality, person-centered care is essential.
