Role Overview
The Community Outreach Specialist plays a vital role in building the organization’s visibility within the community and connecting with potential patients. Serving as the initial contact, this position focuses on educating and engaging individuals about Upward Health's services, guiding them through the enrollment process. The primary outreach is conducted via phone, supplemented by in-person visits and other community engagement methods when necessary. Reporting directly to the Outreach Manager, the Specialist coordinates with the broader team to ensure effective patient connection and program success.
Key Responsibilities and Competencies
- Develop and maintain meaningful relationships with prospective patients to inform and support their healthcare journey.
- Handle personal caseloads using various outreach techniques including telephonic communication and community-based engagement.
- Overcome challenges and objections during outreach while staying motivated and resilient.
- Adapt outreach approaches creatively to meet patient needs and preferences.
- Collaborate effectively within the team, sharing knowledge and contributing towards collective goals.
- Manage time efficiently to balance caseloads and consistently meet outreach objectives.
- Exhibit empathy and cultural sensitivity during all patient interactions.
- Communicate complex service information in clear and persuasive ways.
- Enroll patients smoothly into programs and ensure a seamless onboarding experience.
- Respond constructively to patient concerns to maintain trust and positive outcomes.
- Maintain precise documentation and data management for patient records.
- Utilize community resources to support patient needs beyond the services provided by Upward Health.

- Proficiency with computer systems for communication, documentation, and outreach tracking.
- Flexibility to use multiple outreach formats including phone, in-person visits, and mailed correspondence.
- Fluency in English; Spanish language skills are an advantage.

Compensation and Compliance
