About the Role

The Well Team is dedicated to providing transformative care and support to individuals navigating the NDIS system through its innovative WorkWell™ Model. This model emphasizes personalized and exceptional assistance to help participants achieve their goals and foster greater independence. As a Care Onboarding & Relationship Specialist, you will be the initial point of contact for new participants, setting a positive tone for their care journey through your expertise, compassionate approach, and thorough attention to detail.

Responsibilities

  • Perform comprehensive initial assessments to understand each participant's objectives, preferences, and support needs.
  • Align participants with appropriate support workers and services tailored to their assessed needs and aligned with NDIS funding categories.
  • Manage service agreements, deliver welcome packages, and organize introductory meetings to ensure a seamless transition into care.
  • Accurately record all onboarding related details in the participant management system.
  • Maintain ongoing relationships by regularly checking participant satisfaction and promptly escalating any concerns to the Care Management team.
  • Collaborate closely with support coordinators and care managers to ensure seamless service delivery.

Qualifications and Skills

  • Prior experience in NDIS participant onboarding or a comparable client-centric position.
  • Certificate III or IV in Individual Support, Community Services, or a related discipline.
  • Solid understanding of NDIS plans, funding categories, and participant rights.
  • Excellent verbal and written communication abilities.
  • Possession of or willingness to obtain a Working with Children Check and NDIS Worker Screening Clearance.

Key Performance Indicators

  • Completion rate of participant onboarding processes.
  • Participant satisfaction ratings.
  • Duration until first service delivery post onboarding.
  • Accuracy and compliance in documentation.
  • Participant retention measured at the 90-day milestone.
