Overview

MedCare Pediatric Group is in search of a bilingual Patient Care Coordinator to join our Pediatric Home Health Therapy team in Stafford, TX. This position functions as the main point of communication and coordination between families, therapists, physicians, and referral sources after therapy services commence. The role demands a highly organized, detail-oriented professional dedicated to delivering superior customer experience through effective documentation management, authorization tracking, and daily process handling in a dynamic healthcare environment.

Core Duties

  • Act as the primary liaison for bilingual (English and Spanish) families concerning ongoing therapy services.
  • Facilitate communication among families, therapists, physicians, referral sources, and internal teams.
  • Oversee therapy documentation completion ensuring adherence to agency standards and promptness.
  • Support procurement, tracking, and administration of insurance authorizations and payer prerequisites.
  • Manage coordination of physician orders, care plans, and required clinical documentation.
  • Maintain precise patient records within the electronic medical record (EMR) system.
  • Contribute to quality improvement efforts including documentation reviews and operational enhancements.
  • Identify workflow inefficiencies and partner with leadership to optimize processes and enhance patient access.
  • Deliver exceptional customer service ensuring patients receive timely and coordinated therapy treatments.

Requirements

  • Fluency in English and Spanish, both spoken and written.
  • Outstanding communication, organizational, and customer service capabilities.
  • Ability to effectively prioritize and multitask within a fast-paced setting.
  • Strong attention to detail combined with adept problem-solving skills.
  • Proficiency with Microsoft Office suite and various computer software applications.
  • Preferred experience in healthcare administration, home health, therapy services, or medical office environments.
  • Familiarity with EMR systems, insurance authorization processes, physician orders, and clinical documentation is favored.
  • Working knowledge of home health operations and compliance requirements is a plus.
