About the Role
An exciting opportunity is available for a motivated Band 6 nurse with a background in cardiac nursing to join the Heart Failure service both in inpatient and community settings within Aneurin Bevan University Health Board (ABUHB). This service supports individuals living with heart failure across multiple locations including Ysbyty Aneurin Bevan, Ysbyty Ystrad Mynach, County, Chepstow, Newport, Monmouth, and Nevill Hall Hospital. The role comprises managing patient caseloads alongside Band 7 staff, delivering both face-to-face and remote consultations.
Key Responsibilities
- Contribute professionally to the development and delivery of an integrated, sustainable heart failure service.
- Provide comprehensive, holistic care addressing physical, psychological, and social aspects to support patients living with heart failure.
- Manage and coordinate care for a caseload within a multidisciplinary team, ensuring high standards across varied patient pathways and disease stages.
- Demonstrate proficient clinical assessment skills aimed at optimizing patient medications.
- Offer expert clinical support, education, and training to healthcare professionals involved in heart failure patient care.
- Assist in leading service development and integration of palliative care in line with NICE guidelines, continually evaluating service quality.
- Engage actively in research and promote evidence-based practices, aiming to publish findings collaboratively.
Candidate Requirements
- Registered nurse qualification (RGN/RN level) with graduate education.
- Post-registration cardiac nursing development and experience working with cardiac patients.
- Proven ability to independently manage patient caseloads.
- Essential knowledge and understanding of the cardiac speciality.
- Preferred qualifications include experience in health promotion, managing heart failure patients, masters level education or progress toward it, prescribing capabilities, and teaching experience.
