Job Responsibilities

• Analyze medical records to identify patient diagnoses and treatments.
• Apply ICD-10-CM diagnosis codes accurately to patient records.
• Map diagnoses to Hierarchical Condition Categories (HCC) for risk adjustment.
• Maintain code accuracy and consistency throughout the documentation.
• Perform audits on medical records and documentation to confirm correctness.
• Provide constructive feedback to physicians and healthcare providers regarding coding standards.
• Educate physicians, providers, and clinical teams on compliance with clinical documentation requirements.

Required Skills and Qualifications

• Strong expertise in medical coding principles and practices.
• Exceptional attention to detail to ensure precise coding.
• Dedication to accuracy and adherence to compliance standards.
• Proficient in ICD-10-CM, CPT, HCPCS, and HEDIS CAT II coding systems.
• Certification in CRC, CPC, or CCS is mandatory.
• Minimum of one year experience working as a certified HCC coder.
