Job Responsibilities
- Analyze medical records to identify patient diagnoses and treatments.
- Apply ICD-10-CM diagnosis codes accurately to patient records.
- Map diagnoses to Hierarchical Condition Categories (HCC) for risk adjustment.
- Maintain code accuracy and consistency throughout the documentation.
- Perform audits on medical records and documentation to confirm correctness.
- Provide constructive feedback to physicians and healthcare providers regarding coding standards.
- Educate physicians, providers, and clinical teams on compliance with clinical documentation requirements.
Required Skills and Qualifications
- Strong expertise in medical coding principles and practices.
- Exceptional attention to detail to ensure precise coding.
- Dedication to accuracy and adherence to compliance standards.
- Proficient in ICD-10-CM, CPT, HCPCS, and HEDIS CAT II coding systems.
- Certification in CRC, CPC, or CCS is mandatory.
- Minimum of one year experience working as a certified HCC coder.
