About the Role

The Education Continuity Coordinator (ECC) plays a pivotal role at Sandstone Care, ensuring seamless access to educational rights and services for adolescents and young adults enrolled in the Partial Hospitalization Program (PHP). Working within a framework governed by behavioral health regulations and state educational authorities, the ECC serves as the main liaison among the treatment program, home schools, Local Education Agencies (LEAs), post-secondary institutions, and other relevant educational stakeholders across designated states.

Key Responsibilities

  • Conduct detailed educational assessments for newly admitted clients.
  • Analyze Individualized Education Plans (IEPs) and Section 504 Plans to facilitate and monitor ongoing learning progress.
  • Provide academic and vocational support, including administering assessments where appropriate, setting goals with measurable objectives, and coordinating needs with schools, BOCES, families, and recreational services.
  • Ensure timely and accurate documentation of all educational services provided.
  • Support clients in completing homework and related academic tasks.
  • Maintain effective relationships with schools, families, referral groups, and community resources.
  • Clearly and effectively communicate ideas to diverse audiences.
  • Exhibit sound judgment and emotional stability when making decisions under pressure or stressful conditions.

Qualifications

  • Bachelor’s degree required; Bachelor’s or Master’s degree in Education or Behavioral Health disciplines (such as clinical social work, clinical psychology, marriage and family therapy) is preferred.
  • Eligible for or holding a teaching license in the state of employment.
  • Familiarity with the Individuals with Disabilities Education Act (IDEA).
  • Experience mentoring adolescents and providing academic, vocational, and life skills support.
  • Preferred: Minimum of three years experience working with clients aged 13 to 18.
  • Strong verbal and written communication aptitude.

Compensation and Schedule

  • Hourly wage ranging from $25 to $30, depending on experience and licensure.
  • Full-time schedule, Monday through Friday, with some evening hours required.
  • Work location: Hybrid settings in Colorado and Maryland at Teen & Young Adult Outpatient Treatment Centers.
