Role Overview
The Community Facilitator for the Early Learning Program (ELP) and Hostel will assist the Manager in delivering exceptional daily care for ELP and Hostel residents. Responsibilities involve supervising and safeguarding residents with intellectual disabilities and/or autism, supporting independent living skills, leisure, recreation, sports, career development, and encouraging community participation.
Key Responsibilities
- Collaborate closely with Life Coaches to support ELP residents living independently onsite.
- Coordinate with Community Facilitators and Training Officers to aid Hostel residents in daily living and training routines.
- Organize community support networks and social or recreational activities.
- Maintain accurate records, care plans, and incident logs related to residents and clients.
- Ensure adherence to residents' dietary restrictions and medical needs.
- Guarantee residents' safety within their living environments, maintaining cleanliness and order.
- Monitor health and hygiene standards of residents and clients.
- Develop, review, and monitor Individual Care Plans for ELP and Hostel residents.
- Conduct regular onsite visits to ensure residents coexist harmoniously.
- Address conflicts, emergencies, and requests for assistance promptly.
Training and Development
- Assess individual needs and plan personalized training aligned with care and training plans to achieve person-centered goals.
- Design and implement programs and activities in consultation with staff and allied health professionals.
- Supervise and guide residents in independent living, leisure, recreation, sports, and career progression.
- Maintain and enhance independent living skills of residents whenever feasible.
- Identify and manage risk factors alongside residents and clients.
- Ensure all monitoring and alert systems are functional and reliable.
Family, Community, and Case Management
- Engage with Multi-Disciplinary Teams for regular case reviews and to discuss residents' progress and concerns.
- Establish professional, trusting relationships with family caregivers and community stakeholders like employers, grassroots organizations, volunteers, and befrienders.
- Collaborate with caregivers and social workers to support family work and future care planning.
- Assist residents in resource navigation, financial management, and safe travel arrangements to work, appointments, and activities.
- Provide information about social support services offered by other organizations and community resources.
Qualifications and Skills
- Possess a Degree or Diploma in any relevant discipline.
- Hold a minimum of three years' relevant work experience.
- Demonstrate knowledge and experience in residential care, assisted living, home-based services, or disability sectors.
- Skilled in planning and executing community activities and training programs.
- Proficient with computer applications including MS Word, Excel, and PowerPoint.
- Exhibit strong interpersonal and communication skills and work effectively within a team.
Additional Information
- Willing to commit to a 5-day work week on a rotating shift basis (morning and afternoon shifts), including alternate Saturdays, Sundays, and public holidays.
