Care Navigator

About the Role

Wider Circle is seeking an entry-level Care Navigator to join our Care Delivery Operations team. You'll engage with patients primarily via phone, occasionally in person, to conduct health assessments and intake appointments. Building immediate trust is key, alongside gathering information on factors influencing patient health such as nutrition, transportation, and housing, while maintaining precise and comprehensive call records. This position offers a solid foundation to progress into a Care Advocate role with a defined advancement path.

Key Responsibilities

• Conduct health screenings and intake calls using standardized questionnaires, fostering rapport in 15-20 minute conversations to encourage honest patient responses.
• Handle scheduling directly within the Healthie platform, arranging intake appointments yourself when necessary to ensure continuity, and follow up to maintain a show rate above 60%.
• Explore patient needs through open-ended questions, then coordinate subsequent steps including doctor appointments or Community Health Worker engagements.
• Document all patient information accurately and in real time adhering to privacy regulations (HIPAA), ensuring patient consent for all interactions and completing documentation by shift end.
• Effectively manage daily schedules to meet performance goals and allocate sufficient time for note review between appointments.

Qualifications and Requirements

• Minimum education requirement: High school diploma or equivalent; degrees in Human Services, Public Health, or Behavioral Health are advantageous.
• Bilingual proficiency in English and Spanish or another language is highly valued.
• Prior healthcare, social service, care platform (such as Healthie), or community outreach experience beneficial but not mandatory; comfortable engaging with diverse patient populations by phone.
• Technologically adept, able to work with web and application-based systems, and adapt quickly within dynamic settings.
• Capable of working independently, organizing tasks and documentation effectively while meeting goals remotely with minimal oversight.
• Commit to comprehensive training including call simulations and documentation practice, requiring a minimum 80% final assessment score.
• Successful completion of background check and drug screening before employment.

Physical and Work Environment Requirements

• Ability to sit, stand, and walk regularly to perform discussions and daily activities.
• Good hearing and vision to communicate effectively on phone and in person, and to respond to detailed inquiries.
• Competent in using computers to enter and manage health data concurrently with phone conversations.
• Availability of a stable internet connection and a quiet, private workspace for conducting remote calls.
