About the role
The Royal Borough of Windsor and Maidenhead is a unitary authority in Berkshire, serving a borough with strong economic activity, growing business sectors, and major cultural and tourism landmarks such as Windsor Castle, Legoland, and Ascot Racecourse. This opportunity is for an enthusiastic Social Care Practitioner to join the Hospital Discharge Team.
Role overview
In this post, you will support adults with care needs in hospitals, their own homes, or care environments. The work centers on assessing individual needs, arranging suitable support, and helping people maintain or regain independence and wellbeing.
Key duties
You will assess adults in line with the Care Act 2014, oversee a caseload, and work with individuals, families, and other professionals to plan and coordinate care. The role includes safeguarding, managing risk, creating support plans, and ensuring work is delivered within legal, policy, and budgetary requirements. You will also help the team perform effectively, keep records accurate and up to date, and work in line with the council’s values and governance expectations.
Candidate profile
The ideal applicant will have experience working with vulnerable people, including those with disabilities or complex needs, and will bring empathy and professionalism to the role. Strong written and verbal communication is essential, along with the ability to produce clear reports and build positive relationships with service users and colleagues. You should be confident working both independently and as part of a team, able to adapt to changing priorities and environments, and comfortable using IT systems for casework and documentation. A person-centred approach, commitment to safeguarding and equality, and sound professional judgment are also important.
