Role Overview
The Healthguide will work remotely to strengthen the coordination between patients and their primary care physicians or medical practice teams. This role involves assisting patients in navigating their healthcare journeys, addressing medical issues, enhancing communication with providers, and promoting improved health and wellbeing.
Key Responsibilities
- Collaborate with patients 27 medical practice staff and Guidehealth 27s Case Management team to manage and support a targeted patient population remotely.
- Operate independently while also functioning as a collaborative team extension of the medical practice.
- Support patients in achieving better health results by addressing inquiries, facilitating connections to healthcare providers, helping implement impactful care plans, closing care gaps, escalating medical concerns, and guiding patients to appropriate resources such as specialty referrals and community organizations.
- Maintain precise and timely documentation of patients 27 medical history and current medical challenges in electronic health records (EHR).
- Understand and navigate referral systems and prior authorization processes.
- Build and sustain trusted, professional relationships within assigned medical practices and Guidehealth.
- Meet designated Key Performance Indicators focused on improving patient health outcomes while reducing medical expenses.
- Engage high-risk patients using multiple communication methods including phone calls, texts, patient portals, and emails to schedule and follow up on care.
- Escalate urgent medical issues appropriately to Health Guide Registered Nurses, Primary Care Physician offices, or Guidehealth management.
- Adhere strictly to all security policies protecting Protected Health Information (PHI) and intellectual property maintained by Guidehealth.
- Be prepared for compensated travel (10-15%) to clinical locations around Atlanta, GA, as necessary for patient/provider engagement and retinal eye exams, with training provided.
Qualifications and Skills Required
- Certification or registration as a Medical Assistant or EMT, with at least two years experience.
- Passion for coordinating care and supporting patients with complex medical and social needs.
- Willingness to develop skills in motivational interviewing, active listening, and behavioral change techniques to improve patient engagement.
- Excellent interpersonal abilities emphasizing empathy, patience, professionalism, and respect in all interactions.
- Outstanding communication skills and accuracy in real-time documentation of patient conversations using relevant software.
- Familiarity with medical terminology and an understanding of primary care physician practice operations.
- Knowledge about social determinants that impact health.
- Strong technology skills and the ability to independently navigate multiple platforms simultaneously, including electronic medical records, care management databases, phone systems, and Microsoft Office tools.
- Proficiency with Microsoft Outlook, Word, and Excel.
- Experience with a variety of EHR systems such as eClinicalWorks, EPIC, Athena, and Cerner.
- Ability to manage multiple priorities effectively while maintaining detail-orientation.
- Capability to work autonomously as part of a care team under clinical supervision.
- Proven success working remotely in a professional setting.
