Role Overview

As a Healthguide, you will work closely with clinical teams to strengthen relationships between patients and their primary care providers. Your role will involve assisting patients in navigating their healthcare, addressing concerns, improving communication, and supporting overall health and wellness.

Key Responsibilities

  • Primarily operating remotely, collaborate with medical practice staff and Guidehealth Case Management to manage selected patient populations.
  • Work autonomously while remaining a cooperative team member integrated with medical practices.
  • Support patients in achieving better health outcomes by responding to their inquiries, coordinating care with medical practices, facilitating impactful care plans, closing care gaps, escalating medical issues, and directing patients to appropriate resources including specialty referrals and community organizations.
  • Maintain accurate and timely documentation of patients' medical history and issues in electronic health records (EHR).
  • Have knowledge of referral and prior authorization processes.
  • Build and sustain professional, trusting relationships with assigned medical practices and within Guidehealth.
  • Meet assigned Key Performance Indicators (KPIs) to improve patient health outcomes and reduce medical costs.
  • Use effective communication channels like calls, texts, portals, and emails to engage and schedule high-risk patients.
  • Escalate medical concerns appropriately to nurses, physicians, or leadership.
  • Follow all organizational security policies to safeguard Protected Health Information (PHI) and intellectual property.
  • Occasionally travel (10-15%) to clinical sites in Atlanta, GA for patient/provider engagement and retinal eye exams, with paid training included.

Qualifications and Experience

  • Certified or Registered Medical Assistant or Certified EMT with a minimum of 2 years experience.
  • Passion for coordinating care for patients with complex medical and social needs.
  • Willingness to develop skills in motivational interviewing, critical listening, and behavioral change management to enhance patient engagement.
  • Strong interpersonal skills emphasizing empathy, patience, professionalism, and respect in all interactions.
  • Excellent conversational and real-time patient documentation skills using software tools.
  • Familiarity with medical terminology and primary care practice functions.
  • Understanding of social determinants of health.
  • Proficiency with technology and ability to navigate multiple platforms including EHRs, care management databases, phone systems, and Microsoft Office suite (Outlook, Word, Excel).
  • Experience with various EHR systems like eClinicalWorks, EPIC, Athena, and Cerner.
  • Ability to manage multiple priorities with attention to detail and focus.
  • Capacity to work independently under supervision of licensed clinicians.
  • Demonstrated success in remote work environments.
  • Reliable vehicle and valid driver's license for occasional travel.
  • Maintaining patient confidentiality by using private home office setups and provided headsets.
  • Compliance with organization policies and willingness to perform other assigned duties.
