Unlike fast-paced hospital settings, this role offers a slower tempo, enabling you to work long-term with patients within residential and community contexts, focusing on recovery-oriented care for adults diagnosed with mental illness or dual diagnosis.

Service Overview

Adult Community Clinical Services (ACCS), funded by the Department of Mental Health, provides evidence-based clinical interventions. Individuals served live independently, in their own homes or group settings. Our multidisciplinary, close-knit team supports clients transitioning toward greater independence by enhancing natural supports, skill-building, and personal goal achievement. The team includes clinicians, substance use counselors, nurses, peer and housing counselors, outreach, and direct care staff delivering compassionate, quality care.

Key Responsibilities

• Collaborate with the integrated team to perform comprehensive physical health assessments and facilitate referrals to appropriate community healthcare providers.
• Monitor clients’ medical and psychiatric conditions through observations, interviews, and document reviews, providing recommendations as necessary.
• Provide training and consultative guidance to programs about medical issues, procedures, and health promotion practices.
• Deliver personalized education to clients regarding health, disease processes, wellness, and preventive care.
• Regularly review clinical records to ensure adequate services, including medical interventions, tests, or preventative treatments.
• Oversee the Medication Administration Program ensuring adherence to regulatory standards.

Qualifications and Skills

• Current Registered Nurse license in Massachusetts is mandatory.
• Possession of a valid driver’s license with a satisfactory driving record.
• Strong skills in clinical assessment, documentation, and verbal communication.
• Experience working with adults with mental health or psychiatric disorders is preferred.

Compensation

The base rate for this role is USD 39.95 per hour.
