Role overview
This six-month contract role is for a Project Manager focused on population health work, with the possibility of an extension. The position is fully remote within the United States. The successful candidate will help drive growth by leading projects that improve and optimize operational processes, and will report to the Medicare, Managed Care, and Population Health Program Manager.
What you will do
- Oversee assigned projects from initial concept through final delivery.
- Lead initiatives that support the implementation and optimization of operational workflows.
- Build and maintain project controls for scope, quality, performance, and risk across multiple efforts.
- Coordinate closely with business, clinical, technical, and vendor stakeholders to keep work moving toward on-time completion.
- Develop working relationships with external partners and third-party vendors.
- Allocate and monitor resources to improve delivery efficiency and maximize output.
- Escalate project risks and outcomes to the right management channels when needed.
- Keep communication clear and active across all team members.
- Identify the right resources for each task and actively follow up when needed.
Core responsibilities
- Partner across teams to prepare planning documents and effort estimates.
- Own key project management deliverables such as schedules, charters, and forecasts.
- Manage project operations through activation and transition into operations.
- Remove blockers by driving follow-up and seeking clarity on issues.
- Track and document issues, risks, and dependencies proactively.
- Forecast resources and project costs while supporting the financial cycle.
- Run project team meetings, including note-taking when required, and track issues, risks, milestones, and timelines.
- Deliver weekly status updates.
- Apply PMO or program standards and maintain required project documentation and compliance artifacts.
- Support projects that implement tools and systems for longitudinal patient tracking and care coordination across acute, post-acute, and ambulatory settings.
- Contribute to payer contract modeling and analytics for value-based care negotiations.
- Support health data integration and real-time reporting for clinical and operational use cases.
What we are looking for
- Strong experience delivering EHR-related work and managing several projects end to end.
- Background in healthcare or provider settings, ideally within an enterprise health system.
- Experience working with or overseeing product development teams.
- Familiarity with at least one major acute EMR, with Athena or Cerner preferred.
- Exposure to care coordination or HIE-enabling platforms.
- Experience working with agile or product management teams.
- Ability to translate project goals into meaningful work for clinical and operational end users.
- Solid communication and collaboration skills across technical, business, and clinical groups.
- Comfort with financial tracking, including monitoring project spend.
- Experience with ServiceNow and capital resource allocation.
- Prior work in enterprise health systems is preferred.
- Experience supporting population health initiatives.
- Demonstrated success partnering closely with end users in population health environments.
- Healthcare experience with a working understanding of value-based care, Medicare, and Medicaid.
- Candidates should be prepared to discuss their experience and skills clearly during interviews.
- Applicants must be comfortable leading projects in a remote setting with cameras on, and should be able to explain their project approach and lifecycle management.
