Job description

Role Overview

This position involves supporting clients in their residences by assisting with personal health care and daily health maintenance tasks. Duties include helping with transfers, feeding, mobility, exercises, and other essential care activities as assigned according to the patient's care plan.

Patient Interaction

The role involves interaction with patients of all age groups within clinical settings and requires demonstrating appropriate and respectful conduct toward all patient populations.

Primary Responsibilities

  • Perform assistance tasks within the scope of a home health aide following the established Plan of Care.
  • Effectively communicate with administrative personnel regarding assessments of client and family needs and promptly report any significant changes in the patient's condition.
  • Deliver care per the Home Health Aide Care Plan and maintain accurate documentation of all provided care.
  • Support seamless continued care through collaborative communication with care team members and active engagement in case conferences and team meetings.
  • Adhere to infection control protocols and universal precautions to safeguard both patients and staff during care activities.
  • Commit to ongoing professional development by participating in training sessions and maintaining required competencies as directed.
  • Fulfill agency and licensing board mandated continuing education requirements.

Experience and Qualifications

  • At least one year of experience as a home health aide or certified nursing assistant in a home health environment is preferred; formal training in nursing arts may substitute experience in accordance with relevant state regulations.
  • High school diploma or its equivalent is required.
  • Must hold a valid Home Health Aide Certificate or equivalent certification current and in good standing, meeting or exceeding standards set by Health and Human Services or applicable state guidelines.
  • Successful completion of an HHA or CNA formal training program aligned with established criteria is necessary.
  • Current CPR certification is mandatory.
  • Must possess a valid driver’s license, reliable transportation, and auto liability insurance; responsibility lies with the employee to keep these current to maintain employment eligibility.

Skills and Abilities

  • Strong ability to develop and sustain positive working relationships with staff and leadership.
  • Proficient literacy in English, including reading, writing, and comprehension.

Physical and Environmental Requirements

  • Capability to stand and walk for prolonged periods.
  • The ability to lift or assist transfers of patients up to 50 pounds.
  • Capacity to work effectively under stressful conditions, meet deadlines, make prompt decisions, and address patient needs efficiently.
  • Meet patients' and families' psychosocial needs with sensitivity and care.
  • Good hand-eye coordination and manual dexterity are required.
  • Work in diverse home environments, sometimes under adverse weather conditions.
  • Possible exposure to bloodborne pathogens, bodily fluids, and infectious agents necessitates strict adherence to safety protocols.
  • Availability for occasional emergency calls and flexibility to travel locally.
