Overview
The Population Health Assistant role at Advocate Health involves supporting various confidential population health initiatives. These responsibilities extend to coordinating care gap interventions, conducting research, managing resources, and facilitating communication with a broad range of stakeholders including patients, providers, employers, and insurance companies. The role also encompasses compiling, analyzing, and reporting population health data to inform departmental needs, along with performing administrative tasks to ensure smooth workflow.
Work Schedule and Location
This is a full-time position requiring 40 hours per week, scheduled Monday through Friday from 8:00 AM to 4:30 PM. The work is primarily remote with occasional in-person meetings.
Key Duties
- Manage intake and triage of cases to expedite the Care Coordination process by facilitating medical reviews, verifying eligibility, and collecting necessary clinical and non-clinical data.
- Maintain and organize the team’s library of resources, guidelines, and policies; conduct medical literature reviews as needed.
- Support onboarding for new employer groups in collaboration with nursing and sales teams, ensuring availability of materials and establishing interim reporting processes.
- Administer care gap outreach efforts by generating and validating intervention lists and documenting outreach activities as per protocols.
- Assist in onboarding and training new department staff on information systems, individual health plans, policies, and procedures; educate provider office staff on referrals, authorizations, contracts, and benefit limitations.
- Generate reports and summarize data for internal management and external partners; assist in data monitoring and analysis.
- Coordinate the ordering and stocking of materials that support Population Health activities, including graphic design tailored to branding and business needs.
- Maintain department records, files, logs, and establish additional record-keeping as necessary.
- Perform routine administrative duties such as managing incoming communications, typing correspondence, monitoring office equipment, and ordering supplies.
Qualifications
- High school diploma or equivalent required.
- A minimum of five years’ experience in population health management, utilization management, claims processing, health insurance, clinical care management, or quality assurance is preferred.
- Familiarity with ICD-10, CPT, and HCPCS coding is advantageous.
- Strong knowledge of medical terminology and ability to manage data systematically.
- Excellent organizational skills with keen attention to detail and accuracy.
- Proficiency in Microsoft Office Suite (Excel, PowerPoint, Word) and internet browsers.
- Effective interpersonal and communication skills, both verbal and written, suitable for interacting with diverse individuals.
- Ability to exercise independent judgment, demonstrate problem-solving skills, and work collaboratively within a team environment.
- Self-motivation in prioritizing and completing assigned tasks with confidentiality maintained at all times.
Physical Requirements and Environment
- Must be able to perform physical activities including sitting, standing, walking, lifting (up to 10 lbs occasionally), bending, twisting, crawling, kneeling, climbing, and reaching.
- Continuous keyboard use is required.
- Typically works in a standard office environment using necessary equipment to complete duties.
