Position Overview
The Population Health Administrator is responsible for leading complex, large-scale projects in population health and value-based care. This role involves developing and sustaining enterprise-wide strategies that enhance quality outcomes and financial performance. The incumbent collaborates with cross-functional teams to establish infrastructure and procedures aligned with business objectives within a multifaceted academic health system. They independently manage portfolios including project scoping, redesign, implementation, metric creation, and ongoing monitoring to ensure sustained success.
Key Duties and Responsibilities
- Lead system-wide performance improvement projects related to population health and value-based care initiatives.
- Develop process improvements to meet organizational goals for quality and financial metrics.
- Collaborate with clinical and administrative teams to design and implement infrastructure needed for population health management.
- Analyze extensive data sets to identify trends, issues, and improvement opportunities.
- Prepare and deliver comprehensive reports, presentations, and actionable plans to stakeholders.
- Apply change management and project management best practices to ensure successful project execution.
- Guide teams effectively to meet project objectives.
Qualifications and Skills
- Bachelor’s degree required; Master’s degree preferred.
- Valid California RN license preferred.
- At least 5 years of relevant experience in population health, performance improvement, or value-based care.
- Strong proficiency in Microsoft Office, particularly PowerPoint, Excel, and Visio.
- Demonstrated expertise in data analysis and interpretation of complex health system reports.
- Experience with performance improvement methodologies and quality standards.
- Excellent verbal and written communication skills, capable of engaging diverse audiences.
- Ability to lead teams and manage multiple projects simultaneously.
- Prior experience with the EPIC electronic health record system is beneficial.
- Ability to maintain confidentiality and exercise sound judgment with sensitive information.
