Role Overview

The Patient Engagement Specialist is a vital member of the care team tasked with acquiring new patients and supporting growth initiatives. This role works closely with cardiology practices to identify eligible patients, coordinate with providers, educate patients on program services, answer inquiries, collect relevant information, and arrange initial virtual appointments.

Key Responsibilities

  • Maintain consistent on-site presence at partner cardiology clinics.
  • Provide remote support to additional nearby clinics as required.
  • Pre-screen patients to determine eligibility and alert providers about potential enrollments.
  • Verify patient eligibility and gather feedback from providers about enrollment possibilities.
  • Conduct enrollment and onboarding sessions with patients.
  • Document activities in Karoo Health's Kohere.AI system and partner cardiology systems for accurate tracking and reporting.
  • Enhance patient enrollment by educating patients on Karoo Health's programs and Model of Care to ensure clear understanding of benefits and services.
  • Assist practice providers by enrolling patients and facilitating their transition to the virtual care team for ongoing care.
  • Collaborate with cardiology partners to monitor and share insights on patients' care journeys.
  • Engage with clinic staff regularly to provide updates on patient admissions, readmissions, and emergency visits.
  • Manage incoming patient calls related to enrollment and program engagement.
  • Implement effective strategies to enroll patients appropriately into the program.
  • Deliver in-service trainings and presentations to practice partners about the Karoo Health program.
  • Maintain regular communication with all interdisciplinary team members to share vital information, coordinate tasks, and support enrollment processes.
  • Participate in special projects, initiatives, and other duties as assigned.

Qualifications

  • High school diploma or equivalent required.
  • Background as Community Health Worker/Partner, Medical Assistant, or Licensed Practical Nurse (LPN).
  • Cardiology-related experience is preferred.
  • At least three years of relevant experience in healthcare or physician clinic/practice environments.
  • Basic knowledge of enrollment and billing procedures.
  • Strong interpersonal skills with the ability to quickly build rapport with patients and providers.
  • Comfortable working in group or team settings and integrating well with clinic staff.
  • Confident in explaining new initiatives and programs to patients and other stakeholders.
  • Experience collaborating in teams and coordinating care with external parties.
  • Excellent communication skills including active listening and strong written abilities.
  • Adaptability to ambiguous, fast-paced environments with a solution-driven attitude.
  • Capability to analyze data to inform decision making.
