Its platform helps large provider organizations and health systems perform well in value-based care arrangements by improving patient outcomes, lowering overall care costs, and strengthening coordination, analytics, and disease management. Role Overview
Karoo Health is looking for a compassionate and capable Licensed Clinical Social Worker / Medical Social Worker to support patients and families in a virtual care environment. The role centers on psychosocial assessment, counseling, and care coordination. Candidates should have experience working in healthcare settings and a strong, patient-first approach. Experience in cardiology is preferred. An Arizona license and the ability to work standard business hours from 9 AM to 5 PM Pacific Time are required.
Clinical Services
- Perform detailed psychosocial evaluations to understand the needs of patients and their families.
- Build and carry out treatment plans in partnership with multidisciplinary care teams.
- Handle discharge planning and connect patients with community-based support.
Care Coordination
- Work alongside physicians, nurses, and other clinicians to support coordinated, whole-person care.
- Arrange referrals to community programs, support services, and specialty providers.
- Serve as a communication link between patients, families, and clinical teams.
- Take part in interdisciplinary meetings and case review sessions.
Patient and Family Support
- Offer counseling and emotional support to patients navigating medical conditions and diagnoses.
- Explain medical issues, treatment pathways, and coping strategies to patients and their families.
- Support advance directive planning and conversations related to end-of-life care.
Documentation and Compliance
- Keep clinical records complete, accurate, and up to date in proprietary systems and/or electronic health records.
- Follow HIPAA requirements, applicable state rules, and hospital procedures.
- Finish required assessments within the expected timelines.
Qualifications
- Master's degree in Social Work from a CSWE-accredited program.
- Current Arizona licensure as a Licensed Clinical Social Worker or an equivalent credential.
- Active state professional license in good standing.
Experience
- At least two years of clinical social work experience is preferred.
- Prior experience in medical or healthcare settings is strongly preferred.
- Working knowledge of medical terminology and healthcare systems is needed.
- Experience using electronic health record platforms is preferred.
Skills and Competencies
- Strong ability to assess needs and apply clinical interventions.
- Clear verbal and written communication skills.
- Familiarity with community resources and referral workflows.
- Comfort working with people from diverse cultural and social backgrounds.
- Ability to stay effective in fast-moving, high-pressure situations.
- Availability to work 9 AM to 5 PM Pacific Time.
