About Partners Community Health

Partners Community Health (PCH) is a not-for-profit organization dedicated to consolidating healthcare services tailored to the needs of residents in Mississauga and West Toronto. PCH operates two modern Long-Term Care (LTC) facilities and a community hub in West Mississauga with a total of 632 beds. This initiative represents PCH's comprehensive approach to delivering innovative, inclusive programs and fresh care models. Role Overview and Responsibilities

  • Ensure delivery of high-quality resident care within your assigned resident home area following PCH's policies and accountability standards.
  • Engage with residents and their families through effective communication to involve them in their care whenever appropriate.
  • Support all facets of personal care—addressing physical, emotional, social, and spiritual needs—under supervision of Registered Staff or the Director of Resident Care.
  • Apply therapeutic communication techniques to build respectful and collaborative relationships with residents, their families, and coworkers, honoring each person's dignity and individuality.
  • Provide resident-centered care that respects their preferences, promotes their safety, and acknowledges their abilities and limitations.
  • Collaborate with team members in planning, implementing, and documenting daily care plans tailored to individual needs.
  • Contribute actively to continuous quality improvement initiatives for your assigned home area and its associated care services.
  • Participate in mandatory training sessions and ongoing professional development to stay current with best practices.

Qualifications and Requirements

  • Possess a valid Personal Support Worker Certificate from a recognized institution completed with a minimum of 640 curriculum hours.
  • Prior experience of at least one year in a Long-Term Care setting is advantageous.
  • Familiarity with Ministry of Health and Long-Term Care regulations and standards.
  • Capacity to work cooperatively in a team and adapt to dynamic environments.
  • Proficiency in communication (both written and oral), interpersonal interactions, and computer use.
  • Commitment to practicing with an anti-oppression perspective.
  • Additional requirements include valid First Aid and CPR certification, clear Vulnerable Sector Check dated within six months, completion of 2-step TB test, and flexibility to work weekends and statutory holidays.
  • Employees must provide availability to scheduling teams and work at least two shifts monthly to remain actively employed.

Compensation and Work Conditions

The compensation for this position is $27 per hour. Candidates will be expected to work on a part-time basis with unscheduled shifts allocated based on operational requirements and submitted availability.
