National Continuing Care Coordinator - Behavioral Health

About the role

The National Continuing Care Coordinator supports smooth movement for patients who are stepping down from residential behavioral health treatment into outpatient or community-based care. The role focuses on maintaining continuity, improving recovery outcomes, and coordinating effectively with clinical staff, families, and outside care partners.

What you will do

• Create personalized continuing care plans together with patients, clinical teams, and outreach staff.
• Partner with discharge planners to organize aftercare follow-up.
• Manage discharge and post-discharge coordination for inpatient and residential programs, including referrals to outpatient support.
• Stay in regular contact with therapists, counselors, case managers, and other outpatient providers to keep care aligned.
• Act as a bridge between providers such as primary care and mental health specialists so physical and behavioral care work together.
• Take part in interdisciplinary meetings to review patient progress, barriers, and next steps.
• Work with insurance carriers and support authorization requests for continuing care services.
• Maintain detailed reporting on the performance and effectiveness of community aftercare partners.
• Prepare leadership reports on outcomes, including readmission trends.
• Make sure all records and communication follow HIPAA and other privacy rules.
• Train clinical and case management teams on the value of continuing care and long-term recovery planning.
• Advocate for patient access to needed services across the healthcare system.
• Lead training sessions for facility teams on strong continuing care coordination practices.
• Coordinate and communicate remotely using telehealth and other virtual tools.

What we are looking for

• High school diploma or GED is required; a bachelor’s degree in a related field is preferred.
• At least 3 years of experience in behavioral healthcare, especially in case management or continuing care coordination.
• Excellent interpersonal and communication abilities, with comfort working across diverse populations.
• Strong organization and time management skills for supporting a multi-site organization with different regional patient needs.
• Solid understanding of behavioral health systems, treatment approaches, and community resources across the country.
• Ability to collaborate well in a multidisciplinary team setting.
• Good crisis management and problem-solving skills.
• Must hold a valid driver’s license, maintain a clean driving record, keep auto insurance active, and have access to a car.
