Role Overview
As a Care Navigator at Navi, you will be essential in connecting individuals to life-changing mental health care efficiently. Your goal will be to ensure every person seeking assistance secures an appointment within a week. This role involves assessing needs, crafting tailored care pathways, and supporting clients through to their care transition. Coverage includes some hours between 1 p.m. and 9 p.m. ET on weekdays, plus weekend shifts, with schedules arranged collaboratively to suit both availability and demand.
Responsibilities
- Conduct warm phone intake calls to gather clinical and social information and decide appropriate next steps.
- Match clients to suitable mental health providers based on factors like age, specialty, insurance, language, and cultural competence using Navi’s database and research skills.
- Schedule initial appointments or coordinate with partners to arrange them.
- Perform follow-ups at key points to monitor progress and collect patient feedback.
- Maintain detailed records in the CRM, categorize data for insights, and develop automation aids.
Qualifications and Attributes
- Highly action-driven with the ability to work quickly and efficiently in a fast-paced startup environment.
- Capable of swiftly responding to client needs and remaining engaged until resolution.
- Recognized as a high performer with excellent multitasking abilities.
- People-oriented approach demonstrating empathy, empowerment, and warmth.
- Experience in sales or case management, with a record of motivating others to take action.
- Comfortable navigating complex systems like insurance, provider directories, and waitlists.
- Proficient with technology platforms including CRM tools, Google Workspace, and EMR/EHR systems; experience with Airtable or HubSpot is advantageous.
- Strong oral and written communication skills.
- Authorized to work in the U.S. and available to cover some hours between 9 a.m. and 8 p.m. ET.
Preferred Qualifications
- Two or more years in care navigation, case management, social work, or behavioral health roles.
- Proficiency in Spanish is a plus.
- Relevant credentials such as MSW, MFT, MHC, RN, or related degrees.
